COVID-19 Vaccine Registration

Thank you for your interest in the COVID-19 vaccine.  Please read the instructions below in order to register for an appointment.  If you need any assistance, please contact the Allegany County Department of Health at (585) 268-9250.


Moderna (two doses, ages 18+):

Pfizer (two doses, ages 12+):

  • Tuesday, October 19th (Booster ONLY):
    • You must be eligible for the Pfizer booster vaccination which includes having the 2 doses of Pfizer COVID-19 vaccine at least 6 months prior to this Pfizer booster vaccination clinic. The CDC issued recommendations for a single booster dose of Pfizer COVID-19:
      • individuals 65 years of age or older
      • long-term care facility residents
      • Nursing homes (assisted living facilities, residential care communities, group homes, senior housing)
      • individuals age 50-64 with underlying medical conditions*
      • individuals age 18-49 with underlying medical conditions*, based on individual benefit and risk
      • individuals age 18-64 who are at an increased risk for COVID-19 exposure or transmission because of occupational or institutional setting including essential workers (frontline and non-frontline (; unpaid caregiver of a frail or immunocompromised person; paid and unpaid workers who interact within <6ft of others; and live in a congregate setting (e.g. homeless shelter, correctional facility)

J&J (one dose, ages 18+):

  • Saturday, October 9, 2021, 6:00 a.m. to 5:00 p.m.; Remote Area Medical (RAM) Free Clinic at Genesee Valley Central School – Jaguar Drive, Belmont, NY
    • INSTRUCTIONS: No appointment is necessary as this is a walk in clinic. Please bring your insurance card, a photo ID and wear your mask!

Alfred Pharmacy, Walgreen’s Pharmacy, Fisher’s Pharmacy and Cuba Memorial Hospital also have doses available by appointment only. If interested please call the respective pharmacy.

Parents and legal guardians of any New Yorker, ages 12-17, can enter their student who has received at least their first COVID-19 vaccine dose, for a chance to win a four-year, full-ride scholarship (including tuition, room-and-board, and expenses) to any NYS public college or university (SUNY or CUNY).

What to bring with you to your appointment:

  • Driver’s License or Picture ID with Date of Birth on it
  • Insurance Card

COVID-19 Vaccine Waiting List for 18+

If you are 18 years of age or older, you may add yourself to the waiting list for the COVID-19 vaccine by clicking on the link below. Currently, the eligibility criteria is open to everyone!  Please take note of the type of vaccine that is available as some are approved for only specific age groups (Pfizer 16+, Moderna 18+, Johnson & Johnson 18+).  If you need assistance with completing this form or have any questions, please feel free to contact the Office for the Aging (60+ years old or disabled) at 585-268-9390 for assistance or the Allegany County Department of Health at 585-268-9250.
