COVID-19 Vaccine Information
Thank you for your interest in the COVID-19 vaccine. The vaccine is currently in limited supply and as soon as the Allegany County Department of Health receives vaccine, we will release a registration link.
AT THIS TIME, THE ALLEGANY COUNTY DEPARTMENT OF HEALTH CAN ONLY VACCINATE PHASE 1B ELIGIBLE ESSENTIAL WORKERS.
The Pharmacies and State-Run mass vaccination sites are designated to vaccinate those 65 years of age and older. Here is a direct link to State-Run mass vaccination clinics: https://am-i-eligible.covid19vaccine.health.ny.gov/
Step 1: Determine eligibility and schedule an appointment. Vaccines are available at pharmacies, hospitals and through the Allegany County Department of Health.
At this time, the Allegany County Department of Health is REQUIRED to prioritize and vaccinate ONLY essential workers that fall into Phase 1B, if vaccine is available. Please read the latest phase guidance by visiting this site: https://covid19vaccine.health.ny.gov/phased-distribution-vaccine.
If you determine that you are eligible for a COVID-19 vaccine as an essential worker in Phase 1B, please do not forget to bring proof or identification with you to the clinic (driver’s license, employment badge, letter from your employer, etc.).
If you are not currently eligible in this phase, please be patient. More vaccine will soon become available.

Step 2: Register for your Vaccination. If you see a registration link below, COVID-19 vaccine is available. If you do not see a link below, vaccine is not currently available. YOU MUST BE IN PHASE 1B AS AN ESSENTIAL WORKER TO RECEIVE VACCINATION, please check your eligibility by visiting the link above and reading the guidance. If you are not eligible as an essential worker in Phase 1B going forward, please do not register for a vaccination appointment as you may be turned away.

Once you have a confirmed appointment, you must complete the New York State COVID-19 Vaccine Form. This form can be completed online and you will receive a submission ID, or can be completed at your vaccination site. (We encourage New Yorkers to complete the form ahead of time.) THIS FORM IS NOT A REGISTRATION FOR A VACCINE APPOINTMENT!
Step 4: Bring proof of eligibility to your appointment. When you go to your vaccination site for your appointment, you must bring proof of eligibility. This may include an employee ID card, a letter from an employer or affiliated organization, or a pay stub, depending on the specific priority status. If you are eligible due to age, you should bring a form of ID (like a Driver’s License or passport) that includes your date of birth. You will need to complete a clinical questionnaire form at the time of your appointment.
Step 5: Schedule your second appointment in-person at your vaccination site. You must schedule an appointment to get your second shot at your vaccination site, in-person, immediately after getting your first shot. The Allegany County Department of Health will provide you with information on how to register for your second dose while you are at our clinic. Remember: Both vaccines require two doses for effectiveness.

If you are 65 years of age or older, you may add yourself to the waiting list for the COVID-19 vaccine by clicking on the link below. Currently, only pharmacies are able to provide the COVID-19 vaccine to those 65+. Please be patient as there is a limited supply of the vaccine.
