What is Long Term Care?
Long term care is medical and non-medical support
services needed to improve or maintain ones’ health and/or daily living. Such
services may be provided in an individual’s home or other community –based and
residential settings.
Who are we here to serve?
Long term care services assist:

adults to live independently for as long as possible
· Younger
adults after an injury or health crisis
· Families
of children living with a disability
· Anyone,
regardless of age, living with a disability
